Risks Associated with Sleeping with Earbuds: Expert Advice
Health professionals have issued a stark warning regarding the habitual use of earbuds during sleep, highlighting an increased likelihood of auditory complications, earwax buildup, and potential infections.
Experts have emphasized that extended exposure to sound, particularly at elevated volumes, poses a significant threat to auditory health.
The persistent presence of earbuds within the ear canal not only retains moisture but also disrupts the natural movement of earwax, thereby fostering inflammation.
To mitigate these risks, they recommend reducing volume levels, taking frequent breaks, and refraining from wearing earbuds overnight.
WHO Guidelines on Sound Exposure
The World Health Organization (WHO) has articulated that the potential for hearing impairment is contingent upon the intensity, duration, and frequency of sound exposure.
According to the WHO, sustained exposure to loud sounds can lead to temporary hearing loss or tinnitus, while recurrent exposure may result in irreversible damage to the sensory cells and associated structures within the inner ear.
Notably, the WHO estimates that over one billion individuals aged 12 to 35 are susceptible to hearing loss stemming from hazardous recreational noise, including that from personal listening devices.
The organization advises keeping headphone or earbud volume below 60 percent of the maximum and advocating for regular listening breaks.
In Nigeria, a WHO regional report reveals that the prevalence of significant hearing loss stands at 6.7 percent, with leading contributors including impacted earwax, chronic otitis media, and age-related auditory decline.
A study conducted in Kaduna involved 272 habitual headphone and earphone users and an equivalent number of non-users, revealing that 17.6 percent of users experienced hearing loss compared to 7.4 percent among non-users.
Warning from an ENT Specialist
Dr. Mustapha Lawal, an esteemed ENT surgeon at the National Ear Care Centre in Kaduna, explained that the repercussions of extended earphone usage hinge on both the volume of sound and the duration of exposure.
He noted that symptoms such as tinnitus, dizziness, and atypical sensations in the ears could manifest prior to the onset of more severe hearing issues.
“Long-term hearing loss is a distinct possibility; initially, individuals may notice symptoms like tinnitus and dizziness,” he remarked.
According to Lawal, prolonged exposure can result in a temporary threshold shift, diminishing the sensitivity of one’s hearing.
“Even if the sound levels are not excessively high, extended listening periods can lead to this shift,” he cautioned.
He urged the importance of giving the ears adequate rest, warning that sustained exposure risks inflicting permanent auditory damage. “Once an irreversible permanent threshold shift occurs, recovery becomes unattainable,” he stated.
Furthermore, Lawal highlighted that persistent noise exposure could hinder concentration, communication, and overall quality of life as hearing difficulties impede daily interactions.
Potential for Ear Infections and Wax Buildup
Professor Adoga Agida Samuel, a distinguished expert in Ear, Nose, and Throat as well as Head and Neck Surgery at the Federal University of Health Sciences in Otukpo, Benue State, cautioned against sleeping with earbuds as it may adversely impact ear canal health.
Samuel elaborated that prolonged occlusion of the ear may incite inflammation and infections, with some users possibly experiencing allergic reactions to the materials used in earbuds.
He identified wax retention as a significant concern, wherein accumulated earwax can obstruct the ear canal, leading to conductive hearing loss. “Wax retention develops gradually but poses serious long-term issues as it accumulates,” he noted.
The professor also warned that exposure to sounds exceeding safe thresholds could induce temporary declines in hearing acuity, with sustained exposure potentially resulting in irreversible auditory damage.
“Sound exposure greater than 85 dB over an eight-hour period yields varying immediate effects, including temporary threshold shifts,” he explained.
Samuel further elucidated that continuous sound exposure could cause oxidative damage, releasing chemicals that ultimately lead to hair cell death, culminating in permanent hearing loss.
Heed Symptoms and Maintain Ear Health
Samuel advised anyone experiencing symptoms such as ear pain, significant itching, tinnitus, or diminished hearing capacity to seek medical evaluation promptly.
“Infections may manifest through pain and pronounced itching, which can often be linked to fungal activity,” he stated.
Specialists advocate for rigorous hygiene practices among earbud users, recommending against inserting objects into the ear canal in an attempt to remove wax, and urging individuals to consult with an ENT specialist if symptoms persist.

While using earbuds during sleep does not automatically result in hearing loss, experts caution that prolonged usage, excessive volume, and continuous exposure may significantly heighten the probability of auditory-related complications.
